The Memory Clinic is a comprehensive care management program for people with memory-related disorders.
It works with patients and their caregivers and family, and also provides educational services to the community. The Memory Clinic includes a comprehensive plan with a social work evaluation, including:
connection to community resources,
follow-ups as needed
The management program involves a team of professionals with expertise in memory-related disorders and who are also willing and compassionate in aspects of the patient’s care.
In order to schedule an appointment for a memory evaluation, a primary care physician must refer the patient. The Memory Clinic works in collaboration with the Alzheimer’s Association Palmetto Chapter, University of South Carolina School of Medicine, and University of South Carolina School of Social Work.
